A report has identified shortfalls in the New Zealand Coastguard's systems in the wake of four search and rescue operations which went wrong over a one-year period.

The Transport Accident Investigation Commission (TAIC) investigated the incidents between March 2009 and March 2010 and found the urgency associated with search and rescue work could adversely affect the decision-making processes.

The first incident occurred when the Coastguard's Tutukaka vessel was called to assist a recreational boat in difficulty in Ngunguru Bay, south of Tutukaka.
